Submit Feedback
The Submit Feedback panel allows the user/provider to send messages to
MassHealth via email. Feedback topics can include suggestions, requests for
assistance, general issues, technical problems, security access, information
updates and claims issues.
It is important
to note that Private Health Information such as Member IDs, Social Security
Numbers and anything covered by Health Insurance Portability and
Accountability Act (HIPAA) confidentiality guidelines never be included in
these emails.

Fields marked with a red asterisk (*) are required.

Error Resolution:
If an
error is detected a statement is displayed in the upper portion of the panel.
The statement indicates the problem and the method of correction and is
preceded by a small red circle with a white "x" () in it. The small red circle with a white "x" ()
also appears next to required fields that are in
error.

Enter feedback information on
the Feedback panel in the following manner:
Drop Down List Box
Select values from a drop down
list box for the Subject
field.
Type Values
Type
values in the Message and E-mail address fields.

Click I'm not a robot checkbox to
display the reCAPTCHA challenge pop-up to verify user is not a robot.

This displays the Google reCAPTCHA challenge pop-up. If challenge is not met
successfully, the reCAPTCHA challenge pop-up will refresh with a different challenge
until the user gets it right. Hitting ESC key will close the pop-up and go back to
the original panel with Submit Email button still disabled since the challenge was
not met.

Click Submit Email to send information.

This either displays a Feedback
Confirmation panel or the current Feedback panel is re displayed with an error
message(s). This button will be disabled by default until the reCAPTCHA challenge
has been met successfully.

Click Cancel Service to clear all information that was entered.
|
An empty Feedback panel is displayed.
